Продолжается обсуждение проблемы дифференциальной диагностики конституциональной задержки пубертата (КЗП) и гипогонадотропного гипогонадизма (гипоГ) у мальчиков, что объясняется схожими этиопатогенетическими механизмами и клинической картиной данных состояний.</p></sec><sec><title>Цель</title><p>Цель: уточнить дифференциально-диагностические критерии КЗП и гипоГ у мальчиков.</p></sec><sec><title>Методы</title><p>Методы. В исследование включено 56 мальчиков 14,4±0,7 года с 1 стадией полового развития по Таннер (G1P1-3/объем гонад &lt;3 см3). В исследование не вошли пациенты с гипергонадотропным гипогонадизмом (гиперГ), получающие лечение половыми стероидами или гонадотропинами в предшествующие 12 мес, пациенты с эндокринными/соматическими заболеваниями, влияющими на половое развитие. При первичном обращении оценивали антропометрические показатели, костный возраст, объем гонад, концентрацию гормонов в сыворотке крови, результаты диагностических тестов с аналогом гонадотропин-рилизинг-гормона (ГнРГ) и хорионическим гонадотропином человека (ХГч). Критерием гипоГ считали объем яичек &lt;3 см3 по истечению двух лет наблюдения. В зависимости от данного критерия, пациентов разделили на две группы: КЗП (n=50) и гипоГ (n=6).</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты с КЗП, в отличие от мальчиков с гипоГ, исходно значимо больше отличались от целевого роста (Me SDS –1,8 и –0,4 соответственно, р=0,02), имели значимо меньший костный возраст (Ме SDS –2,5 против –0,2, р=0,03), значимо больший объем гонад (Ме 1,9 см3 и 0,5 см3, p=0,0003) и концентрацию в сыворотке крови ингибина В (Ме 142,3 и 31,3 пг/мл, p=0,00009), лютеинизирующего гормона (ЛГ) (Ме 1,1 и 0,1 мЕд/мл, p=0,0002), фолликулостимулирующего гормона (ФСГ) (Ме 1,9 и 0,2 МЕ/л, p=0,00007) и max ЛГ (Ме 18,9 и 0,6 мЕд/мл, p=0,00007), значимо больший коэффициент max ЛГ/ФСГ (Ме 2,3 и 0,4, p=0,0002) по результатам теста с аналогом ГнРГ и Δ тестостерона (Ме 14,4 и 1,1 нмоль/л, p=0,0001) по данным теста с ХГч. При оценке дискриминационной способности методов дифференциальной диагностики КЗП и гипоГ, базальная концентрация ЛГ ≥0,3 мЕд/л продемонстрировала чувствительность 86%, специфичность 100%, коэффициент max ЛГ/ФСГ ≥1 – чувствительность 92%, специфичность 100%, Δ тестостерона ≥2,7 нмоль/л – чувствительность 98%, специфичность 100%. При этом, max ЛГ ≥3,5 мЕд/мл, ФСГ ≥0,5 МЕ/л, ингибин В ≥58 пг/мл имели 100% чувствительность и специфичность в диагностике КЗП у мальчиков.</p></sec><sec><title>Вывод</title><p>Вывод. Таким образом, базальное содержание ингибина В ≥58 пг/мл, ФСГ ≥0,5 МЕ/л, ЛГ ≥0,3 мМЕ/мл, так же, как и max ЛГ ≥3,5 мМЕ/мл, коэффициент max ЛГ/ФСГ ≥1,0 по результатам теста с аналогом ГнРГ и Δ тестостерона ≥2,7 нмоль/л по результатам теста с ХГч являются высокоточными дифференциально-диагностическими критериями КЗП и гипоГ у допубертатных мальчиков.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND. The problem of differential diagnosis of constitutional delay of puberty/CDP and hypogonadotropic hypogonadism/HH in boys is discussed, as boys have similar genetic mechanisms and appearance.</p></sec><sec><title>AIMS</title><p>AIMS: to determine accuracy of the criteria for the differential diagnosis of CDP and HH.</p></sec><sec><title>MATERIALS</title><p>MATERIALS. The study included 56 boys 14.4±0.7 years old with delayed puberty (G1P1-3/testicular volume &lt;3 сm3). We excluded patients with hypergonadotropic hypogonadism, treated with sex steroids or gonadotropins for 12 months, with endocrine/somatic diseases affecting puberty. At the first visit, we evaluated anthropometric data, bone age, testicular volume, hormones and the results of the gonadotropin-releasing hormone test (GnRH) agonist test and the human chorionic gonadotropin test (hCG) test. The HH was defined by a testicular volume &lt;3 сm3 after 2 years follow-up. The patients were divided into two groups: the first group with CDP and testicles ≥3 cm3 (n=50) and the second group with HH and testicles &lt;3 cm3 (n=6).</p></sec><sec><title>RESULTS</title><p>RESULTS. At the first visit in boys with CDP corrected target height was less (Me SDS –1.8 vs –0,4, р=0.02), bone age was less (Ме SDS –2.5 vs –0.2 р=0.03), testicular volume was more (Ме 1.9 vs 0.5, p=0.0003), hormones were significantly higher, such as LH (Ме 1.1 vs 0.1 mIU/ml, p=0.0002), FSH (Ме 1.9 vs 0.2 IU/l, p=0.00007), inhibin B (Ме 142.3 vs 31.3 pg/ml, p=0.00009), max LH (Ме 18.9 vs 0.6 mIU/ml, p=0.00007), max LH/FSH (Ме 2.3 vs 0.4, p=0.0002) on the GnRH agonist test and Δ testosterone (Ме 14.4 vs 1.1 nmol/l, p=0.0001) on the hCG test than in boys with HH. The LH ≥0.3 mIU/ml had 86% sensitivity, 100% specificity; max LH/FSH ≥1 – 92% sensitivity, 100% specificity; Δ testosterone ≥2.7 nmol/l on the hCG test – 98% sensitivity, 100% specificity for differential diagnosis of CDP and HH in boys. However, max LH ≥3.5 mIU/ml on the GnRH agonist test, FSH ≥0.5 IU/l, inhibin B ≥58 pg/ml had 100% sensitivity and specificity for diagnosis of CDP.</p></sec><sec><title>CONCLUSIONS</title><p>CONCLUSIONS.
