В прошлом степень йодной недостаточности определяли по содержанию йода в различных объектах внешней среды и не учитывали величину потребления йода человеком. В современных условиях вместо определения содержания йода в воде и почве было рекомендовано определять его в моче. Известно, что более 80% йода выводится из организма почками и, следовательно, концентрация йода в моче достаточно точно отражает величину его потребления с пищей. Ранее этот метод оценки степени йодной недостаточности в России не использовался. Целью работы явились изучение распространенности йодной недостаточности на территории России, апробация метода определения концентрации йода в моче и внедрение его в практику.</p><p>Концентрацию йода определяли в разовой порции мочи церийарсенитовым методом.</p><p>Исследования, проведенные в 25 регионах России на детской популяции (10 853 человека), обнаружили на всех исследованных территориях йодную недостаточность разной степени выраженности. Фактическое потребление йода населением данной возрастной группы составляет в среднем 40—80 мкг/сут при норме, определенной ВОЗ, в 150 мкг/сут. Наиболее низкие концентрации йода выявлены у жителей сельской местности (медиана концентрации йода в моче 30—58 мкг/л) по сравнению с более урбанизированными районами — городом, районным центром (медиана 50— 78 мкг/л при норме более 100 мкг/л).</p><p>Дефицит йода был более выражен в регионах восточной части России, где преобладала йодная недостаточность средней степени (30— 52 мкг/л), а в отдельных районах — тяжелая степень йодного дефицита (16—20 мкг/л).</p><p>Дополнительное регулярное введение 100 мкг калия йодида позволяет нормализовать уровень медианы концентрации йода в моче и значительно уменьшить количество детей, имеющих низкую концентрацию йода в моче.</p><sec><title> </title><p> </p></sec></abstract><trans-abstract xml:lang="en"><p>Development of new methods of investigation modified the approaches to epidemiological analysis of diseases associated with iodine deficiency. Iodine deficiency was evaluated by the level of iodine in environmental objects, while iodine consumption by man was neglected. Today it is recommended to measure urinary iodine excretion Instead of measuring it in water and soil. More than 80% iodine is excreted through the kidneys, and therefore, iodine concentration in the urine reflects its consumption with food. This method of evaluating iodine deficiency has not been used in Russia before.</p><p>We evaluated the prevalence of iodine deficiency in Russia, tried a method for measuring urinary iodine concentrations, and introduced it into practice. Iodine was measured in a portion of urine by the cerium arsenite method.</p><p>Studies were carried out in 10853 children in 25 regions of Russia. Iodine deficiency of different severity was detected in all regions. Actual iodine consumption by children is 40—80 mcg/day, while the WHO-approved norm is 150 mcg/day. The lowest iodine concentrations were observed in the rural residents (30— 58 mcg/liter); in the urban residents the median of iodine concentration was higher (50—78 mcg/liter, vs. the norm — 100 mcg/liter).</p><p>Iodine deficiency was more expressed in Eastern regions of Russia, where medium iodine deficiency predominated (30—52 mcg/liter), and in some regions severe iodine deficiency (16—20 mcg/liter) was observed.
