Наличие у бигуанидов побочных действий часто затрудняет длительное их применение, особенно у больных в пожилом возрасте, имеющих в анамнезе хронические заболевания легких, сердца, почек и печени. В таких случаях целесообразно назначение аноректических препаратов, одновременно оказывающих влияние и на углеводный обмен. Одним из таких препаратов является п-этил-а-метил-3-флюорометилфенэтиламина гидрохлорид (минифаж, пондерал), оказывающий стимулирующее действие на центральную нервную систему. Фенфлюрамин уменьшает запас и синтез моноаминов (5-гидрокситриптамина) в диэнцефальных отделах мозга, за счет чего наблюдается уменьшение потребности в пище, снижение массы тела. Ряд исследователей показали, что гипогликемизирующее действие препарата происходит за счет периферического действия. Фенфлюрамин влияет на экстронейрональное окисление моноамеров и уменьшает уровень триптофана в крови. В экспериментах на крысах подтвердилось, что данный эффект препарата осуществляется за счет улучшения чувствительности периферических тканей к инсулину, в особенности мышц. Некоторые исследователи наблюдали при лечении ферфлюрамuном снижение уровня жирных кислот и холестерина в плазме. Однако вопрос о влиянии препарата на остаточную секрецию С-пептида b-клеток поджелудочной железы в процессе лечения ферфлюромином в доступной нам литературе не освещен. В ходе нашего исследования 26 пациентов с ожирением (контрольную группу составили 18 здоровых испытуемых, на имевших ранее нарушение толерантности к глюкозе) было показано, что фенфлюрамин восстанавливает I фазу секреции инсулина на фоне внутривенной нагрузки с глюкагоном или глюкозой, а компенсация сахарного диабета на фоне лечения фенфлюрамином связана, по-видимому, не только с прямым стимулирующим действием на b-клетки островков Лангерганса, но и с его периферическим действием.</p></abstract><trans-abstract xml:lang="en"><p>With obesity-related diabetes, it is possible to use biguanides, which have a slight anorectic effect. The presence of side effects in biguanides often complicates their long-term use, especially in elderly patients who have a history of chronic diseases of the lungs, heart, kidneys, and liver. In such cases, it is advisable to prescribe anorectic drugs that simultaneously affect carbohydrate metabolism. One of these drugs is p-ethyl-a-methyl-3-fluoromethylphenethylamine hydrochloride (minifage, ponderal), which has a stimulating effect on the central nervous system. Fenfluramine reduces the supply and synthesis of monoamines (5-hydroxytryptamine) in the diencephalic regions of the brain, due to which there is a decrease in food requirements and a decrease in body weight. A number of researchers have shown that the hypoglycemic effect of the drug occurs due to peripheral action. Fenfluramine affects the extraneuronal oxidation of monoamins and reduces the level of tryptophan in the blood. T.
