Распространенность вторичного гиперпаратиреоза (ВГПТ) зависит от тактики его контроля и целевого уровня ПТГ. Для строго обоснованного выбора целей и тактики коррекции ВГПТ данных недостаточно.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Ретроспективное когортное многоцентровое исследование проведено для оценки результатов применения этелкальцетида в условиях реальной клинической практики. Гипотезой стало предположение, что этелкальцетид будет через полгода эффективно снижать ПТГ, а исходные характеристики МКН-ХБП являются предикторами достижения целевого диапазона.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. В ретроспективное когортное исследование включены 302 пациента, получавшие этелкальцетид в 20 отделениях диализа Санкт-Петербурга.</p><p>Первичная конечная точка — доля пациентов, достигших целевого диапазона через 6 мес (300–599 пг/мл); вторичная — абсолютное и относительное снижение ПТГ. Сравнивали результаты в подгруппах с исходным ПТГ (&lt;600, 600–1000, &gt;1000 пг/мл) и с признаками автономии околощитовидных желез (ОЩЖ) и без них.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Среди включенных пациентов в возрасте 56 (12) лет (мужчин 61%) при медиане срока диализа 36 (Q1–Q3 23–55) месяцев при исходном ПТГ 729 (548–957) пг/мл и при исходной кальциемии и фосфатемии 2,35 (0,25) и 1,97 (0,47) ммоль/л соответственно доля достигших целевого диапазона ПТГ при наличии признаков автономии ОЩЖ составила 48,0% vs. 85,6% в отсутствие автономии (p&lt;0,001) в подгруппе с исходным ПТГ 600–1000 пг/мл. Соответствующее соотношение для пациентов с ПТГ выше 1000 пг/мл составляет 25,0% vs. 57,9% (p=0,016). Снижение ПТГ на 50% зафиксировано у 47,4% всей группы; у 42,9% пациентов с исходным ПТГ&lt;600 пг/мл, у 59,1% пациентов с исходным ПТГ 600–1000 пг/мл и только у 23,6% пациентов с более выраженным ВГПТ (p&lt;0,001).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Применение этелкальцетида приводило к снижению уровня ПТГ у большинства пациентов на гемодиализе. При умеренном ВГПТ (ПТГ 600–1000 пг/мл) достичь целевого уровня ПТГ можно у большинства пациентов, но при более высоком исходном уровне ПТГ результат снижается. Признаки автономии околощитовидных желез связаны с большим риском недостаточного эффекта этелкальцетида, чем исходный уровень ПТГ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: The prevalence of SHPT depends on control strategy and target level of PTH. There is insufficient data for a justified choice of goals and strategy for the SHPT correction.</p></sec><sec><title>AIM</title><p>AIM: A retrospective, multicenter, cohort study was conducted to assess the ethelcalcetide efficacy after six months in real world practice. The study sought to identify whether MBD-CKD characteristics are predictive of achieving the goals.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: A study included 302 patients in 20 dialysis units in St. Petersburg. The primary endpoint was the proportion of patients who reached the target of PTH (300–599 pg/ml). The secondary endpoint was an absolute and relative decrease in PTH. The results were compared in subgroups with PTH (&lt;600, 600–1000, ≥1000 pg/ml) and with and without signs of parathyroid authonomy.</p></sec><sec><title>RESULTS</title><p>RESULTS: In patients aged 56 (12) years (men — 61%) with a dialysis duration of 36 (Q1-Q3 23-55) months with baseline PTH level of 729 (548–957) pg/ml and with calcemia and phosphatemia 2.35 (0.25) and 1.97 (0.47) mM/L, the proportion of success was 49.7%. It is more expected for the PTH of 600–1000 pg/ml compared with PTH&gt;1000 pg/ml: 79% vs. 36%, p&lt;0.001. In presence of signs of parathyroid authonomy, 48% reached the target range vs. 86% in its absence (p&lt;0.001) in subgroup with PTH of 600–1000 pg/ml. For patients with PTH above 1000 pg/ml, the proportion was 25% vs. 58% (p=0.016). The signs of parathyroid authonomy was the most significant independent risk for target achieving (OR=0.3 [95% CI 0.13; 0.68], p=0.004)</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: When transferring patients from cinacalcet to etelcalcetide, no new side effects or drug intolerance were noted. In moderate SHPT (PTH 600–1000 pg/ml) it is possible to achieve the target in most patients, but with higher level, the result worsened. The signs of parathyroid authonomy predicts the insufficient effect better than high baseline PTH level.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>вторичный
