Комбинированное лечение дифференцированного рака щитовидной железы (ДРЩЖ) может оказывать влияние на репродуктивное здоровье пациентов, в частности на овариальный резерв (ОР) женщин детородного возраста. Однако знаний в этой области все еще недостаточно для создания единых рекомендаций и алгоритма ведения данной когорты пациенток, исходя из их текущего репродуктивного статуса и желания реализовать репродуктивный потенциал.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Динамическая оценка функции яичников и ОР с использованием антимюллерова гормона (АМГ), фолликулостимулирующего гормона (ФСГ), лютеинизирующего гормона (ЛГ) и эстрадиола (Е2) у женщин репродуктивного возраста, получающих комбинированное лечение по поводу ДРЩЖ.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. В одноцентровом проспективном несравнительном исследовании проанализированы клинико-морфологические, анамнестические и лабораторные параметры у пациенток, получающих комбинированное лечение по поводу ДРЩЖ. Уровни АМГ, ФСГ, ЛГ и Е2 определены на 3-й день менструального цикла в динамике — после хирургического лечения не позднее чем за месяц до радиойодтерапии (РЙТ), а также через 3 и 6 месяцев после РЙТ на фоне супрессивной терапии.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В исследование включено 39 женщин в возрасте от 18 до 40 лет с ДРЩЖ с медианой возраста 32 года [27; 37], проходящих комбинированное лечение по поводу основного заболевания. Частота транзиторного нарушения менструального цикла после оперативного лечения составила 18%, а после РЙТ — 38%. Большинство пациенток, по результатам послеоперационной стратификации риска рецидива заболевания, относилось к группе промежуточного риска (69%). В качестве подготовки к РЙТ 72% пациенток получали отмену левотироксина натрия сроком на 4 недели. Средняя активность 131I составила 3720 МБк [3050; 3838]. Уровни ФСГ и ЛГ до лечения и в процессе динамического наблюдения значимо не менялись (Р=NS). Уровень Е2 значимо снижался через 3 мес. после терапии при сравнении с уровнем до РЙТ (Р&lt;0,010), но через 6 мес. повышался практически до исходных значений (Р=NS). Уровень АМГ значимо снижался через 3 и 6 мес. после РЙТ по сравнению с исходными значениями (Р&lt;0,001). Медиана АМГ до лечения составила 4,10 нг/мл [2,34; 5,82], наименьший уровень АМГ отмечен через 3 мес. — 2,09 нг/мл [1,05; 3,05], а через 6 мес. зафиксировано его небольшое повышение — 2,31 нг/мл [1,42; 3,37]. У 29% пациенток уровень АМГ снизился ниже референсных значений через 3 мес. Предиктором снижения уровня АМГ ниже 1,2 нг/мл (отражает сниженный ОР) через 3 мес. после РЙТ стал возраст женщины на момент начала радионуклидного лечения, с помощью индекса Юдена была определена отрезная точка, равная 31 году.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Уровень АМГ значимо снижается после РЙТ, как второго этапа комбинированного лечения ДРЩЖ, что указывает на влияние проводимой терапии на ОР, при этом возраст на момент РЙТ является основным предиктором снижения АМГ ниже 1,2 нг/мл через 3 мес.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Combined treatment of differentiated thyroid cancer (DTC) may have an impact on the reproductive health of patients, in particular on the ovarian reserve (OR) of childbearing-age women. However, knowledge in this area is still insufficient to create general recommendations and an algorithm for managing this cohort of patients based on their current reproductive status and desire to realize their reproductive potential.</p></sec><sec><title>AIM</title><p>AIM: To assess ovarian function and OR using anti-Müllerian hormone (AMH) and follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2) in dynamics in the early follicular phase in women of reproductive age receiving combined treatment for DTC.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: In a single-center prospective non-comparative study, the clinical and morphological, anamnestic and laboratory parameters of patients receiving combined treatment for DTC were analyzed. The levels of AMH, FSH, LH and E2 were determined in dynamics – after surgical treatment but no later than one month before radioiodine therapy (RAIT), as well as 3 and 6 months after RAIT on the background of suppressive therapy.</p></sec><sec><title>RESULTS</title><p>RESULTS: A total of 39 women aged 18 to 40 years with a median age of 32 years [27; 37] undergoing combined treatment for DTC were enrolled in the study. The frequency of transient menstrual cycle disturbances after surgery was 18%, and after RAIT — 38%. According to the post-operative DTC status the majority of patients belonged to ATA intermediate-risk group (69%). In addition, 72% of patients received thyroid hormone withdrawal for a period of 4 weeks as a preparation for RAIT. The average activity of 131I was 3720 MBq [3050; 3838]. The levels of FSH and LH did not differ significantly before and after RAIT (R=NS). The level of E2 decreased significantly 3 months after RAIT (P&lt;0.010), further increasing in 6 months to almost the initial values (P=NS). The level of AMH decreased significantly 3 and 6 months after RAIT compared with baseline values (P&lt;0.001). The median AMH before the treatment was 4.10 ng/ml [2.34; 5.82], the nadir of AMH was observed after 3 months — 2.09 ng/ml [1.05; 3.05], and after 6 months AMH increased to 2.31 ng/ml [1.42; 3.37]. In 29% of patients, the AMH level decreased below the reference after 3 months. The predictor of AMH level below 1.2 ng/ml (reflecting reduced OR) 3 months after RAIT was the patient’s age before RAIT.
