Дифференциальная диагностика образований надпочечников (аденом, адренокортикального рака (АКР), феохромоцитом) остается сложной задачей из-за схожести их характеристик при стандартной компьютерной томографии (КТ). Двухэнергетическая КТ (ДЭКТ) обладает потенциалом для улучшения дифференциации за счет анализа спектральных характеристик, однако ее роль в диагностике образований надпочечников изучена не в полной мере.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Исследование диагностической ценности характеристик ДЭКТ в дифференциальной диагностике образований надпочечников.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Проведено одноцентровое ретроспективное исследование (данные ДЭКТ за 2023–2024 гг.). Анализировали параметры монохроматических изображений 40, 70 и 80 кэВ, карт разложения вода-йод, жир-йод и вода-жир. Статистический анализ включал тесты Краскела-Уоллиса, Манна-Уитни, ROC-анализ.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Выборка включала 74 пациента с данными ДЭКТ (медианный возраст — 46,0 лет, межквартильный интервал (39,3–57,9); 48 женщин). Наибольшая дискриминативная способность для дифференциации АКР и аденом высокой плотности выявлена у среднего значения рентгеновской плотности изображений 70 кэВ в отсроченной фазе (AUC=0,92; 95% доверительный интервал (ДИ): 0,82–0,99; p=0,001), а для АКР и феохромоцитом — у стандартного отклонения концентрации воды на карте вода-жир в отсроченной фазе (AUC=0,89; 95% ДИ: 0,79–0,97; p=0,001). Для дифференциации аденом и феохромоцитом ключевым признаком стал максимум рентгеновской плотности на изображении 70 кэВ в отсроченной фазе (AUC=0,82; 95% ДИ: 0,72-0,92; p&lt;0,001).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. ДЭКТ демонстрирует высокую диагностическую точность в дифференциации образований надпочечников.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Adrenal lesion characterization remains a diagnostic challenge due to overlapping imaging features. Dual-energy CT (DECT) shows promise for identifying image-based biomarkers but requires further validation.</p></sec><sec><title>AIM</title><p>AIM: To evaluate the diagnostic utility of DECT in differentiating adrenal lesions.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: This retrospective single-center study analyzed DECT data. Parameters from 40/70/80 keV virtual monoenergetic images and water-iodine/fat-iodine/water-fat decomposition maps were evaluated. Statistical analysis included Kruskal-Wallis, Mann-Whitney U, ROC tests.</p></sec><sec><title>RESULTS</title><p>RESULTS: The cohort included 74 patients (median age 46.0 years (39.3–57.9), 48 women). For ACC and adenomas differentiation, mean 70 keV delayed phase attenuation showed the highest discriminative value (AUC=0.92; 95% CI: 0.81–1.00; p=0.001). For ACC and pheochromocytomas, standard deviation delayed phase water concentration (water-fat maps) achieved the highest diagnostic performance (AUC=0.89; 95% CI: 0.79–0.97; p=0.001). Maximum 70 keV delayed phase attenuation best distinguished adenomas from pheochromocytomas (AUC=0.82; 95% CI: 0.71–0.92; p&lt;0.001).</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: DECT has proven its diagnostic value in the differential diagnosis of adrenal lesions.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>двухэнергетическая компьютерная
