Неонатальный сахарный диабет (НСД) – редкое заболевание, в большинстве случаев ассоциированное с активирующими мутациями в гене KCNJ11, кодирующем КИР6.2 субъединицу АТФ-зависимых К-каналов. Сочетание НСД с эпилепсией и задержкой психомоторного развития получило название DEND-синдром (developmental delay, epilepsy и neonatal diabetes).</p></sec><sec><title>Цель</title><p>Цель. Определить наиболее оптимальный метод диагностики и лечения пациента с НСД.</p></sec><sec><title>Методs и результаты</title><p>Методs и результаты. Мы приводим описание 9 летней пациентки, страдающей НСД с 3х месяцев жизни, впервые поступившей в нашу клинику в декабре 2015 года в связи с частыми эпизодами гипергликемии и судорожными приступами по типу абсансов длительностью от 2 до 4 минут. С момента диагностики заболевания пациентке была назначена инсулинотерапия, которую она получала до 9 месяцев жизни, затем до января 2015 года были назначены пероральные сахароснижающие препараты, однако в связи с персистирующей гипергликемией и высоким уровнем гликированного гемоглобина (10,6%) далее вновь была инициирована инсулинотерапия. При осмотре в нашей клинике: рост и вес пациентки соответствовали возрасту. При лабораторном обследовании было выявлено повышение уровня гликемии до 200-300 мг/дл и гликированного гемоглобина до 10,3%. Также отмечалась умеренная задержка умственного развития с IQ - 66. При молекулярно-генетическом исследовании была выявлена гетерозиготная мутация в гене KCNJ11, что позволило диагностировать DEND-синдром и назначить лечение глибенкламидом (3,5 мг) – 8 таблеток в день. Кроме того, были рекомендованы занятия, стимулирующие ментальное развитие, и чтение до 4-5 часов в день.</p></sec><sec><title>Заключение</title><p>Заключение. Идентификация мутаций в гене KCNJ11 у пациентов с НСД позволяет определить тактику дальнейшего ведения таких больных, в частности, является основанием для назначения пероральных сахароснижающих препаратов, что приводит к улучшению качества жизни пациентов и , возможно, оказывает положительное влияние на течение эпилепсии. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Neonatal diabetes is a rare disease and it is frequently caused by a mutation in the KCNJ11 gene, which encodes the Kir6.2 subunit of the ATP-sensitive potassium channel. If the neonatal diabetes is associated with epilepsy and developmental delay, then the diagnosis is of DEND syndrome (developmental delay, epilepsy and neonatal diabetes).</p></sec><sec><title>Aim</title><p>Aim. To determine which are the best methods of diagnosis and treatment for a child with neonatal diabetes.</p></sec><sec><title>Methods and results</title><p>Methods and results. We present the case of a 9 years old girl, diagnosed with neonatal diabetes at age 3 months, who was first admitted to our clinic in December 2015 for frequent episodes of hyperglycemia at home and absence seizure lasting 2-4 minutes, suggesting minor epilepsy. The patient was treated with insulin from the moment of diagnosis until age 9 months, then with oral antidiabetic agents until January 2015, when she started again the insulin therapy in the context of persistent hyperglycemia and a level of glycated hemoglobin (HbA1c) of 10.6%. Her physical examination revealed height and weight according to age, with stable vital signs. The laboratory findings were all unremarkable, except for blood glucose values of 200-300 mg/dl and HbA1c level of 10.3%. The patient also had moderate mental delay, with an IQ of 66. The genetic testing for neonatal diabetes revealed a heterozygous mutation in KCNJ11 gene, so the diagnosis was of DEND syndrome. We initiated the treatment with glibenclamide 3.5 mg, 8 tablets/day and we recommended cognitive functions’ stimulation with exercises and reading 4-5 hours/day.</p></sec><sec><title>Conclusions</title><p>Conclusions. The genetic testing for the identification of a mutation in KCNJ11 gene has an important impact on the therapeutic approach in children with neonatal diabetes, as there is the possibility to replace the insulin therapy with antidiabetic oral agents, therefore improving the quality of life and possibly the epilepsy seizures.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>неонатальный сахарный диабет</kwd><kwd>DEND синдром</kwd></kwd-group><kwd-group xml:lang="en"><kwd>neonatal diabetes</kwd><kwd>DEND syndrome</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title></ref-list><fn-group><fn fn-type="conflict"><p>The authors declare that there are no conflicts of interest present.</p></fn></fn-group></back></article>
