В 27 лет после повторной операции по поводу рецидива заболевания гистологически установлен диагноз ПРЩЖ. У мальчика в возрасте 7 лет, по данным УЗИ щитовидной железы, заподозрен многоузловой зоб, проведена тотальная тиреоидэктомия. При гистологическом исследовании операционного материала выявлена инкапсулированная папиллярная карцинома. Ни сам ребенок, ни его отец не подвергались воздействию радиационного облучения или химиотерапии до момента диагностики ПРЩЖ. Для уточнения этиологии заболевания проведен молекулярно-генетический анализ методом секвенирования следующего поколения (NGS). У пробанда и его родителя в экзоне 4 гена DICER1 выявлена гетерозиготная делеция тимина в позиции 380, что приводило к сдвигу рамки считывания с образованием преждевременного стоп-кодона (c.380delT p.L127QfsX3).</p></abstract><trans-abstract xml:lang="en"><p>Here, we report a clinical case of isolated papillary thyroid cancer associated with a germline DICER1 gene mutation in a boy and his father. The father underwent surgery for a euthyroid multinodular goiter at the age of 7 and 9 years. On examination at the age of 27 years, he was diagnosed with papillary thyroid cancer. At the age of 7 years, the boy was suspected of having a multinodular goiter (based on thyroid ultrasonography findings); he underwent total thyroidectomy. A histological examination of the surgical material revealed encapsulated papillary carcinoma. Neither boy nor his father had been exposed to radiation or chemotherapy before the diagnosis of papillary thyroid cancer. To clarify the etiology of disease, a molecular genetic testing was performed using next-generation sequencing (NGS).
