Молекулярно-генетические и клинические варианты MODY2 и MODY3 у детей в России.

Аннотация

Цель исследования — изучить молекулярно-генетические и клинические особенности сахарного диабета типа MODY2 и MODY3 у детей.

Материал и методы. Молекулярно-генетическое исследование генов GCK и HNF1α было проведено у 169 человек с нарушениями углеводного обмена, диагностированными в возрасте до 18 лет и клинически интерпретироваными как проявления MODY. У 62 пациентов с верифицированным MODY2 и 18 пациентов с MODY3 проведен анализ клинических данных при диагностике нарушений углеводного обмена и при динамическом наблюдении.

Результаты. Отношение MODY2 к MODY3 составило 3,4:1. Нарушения углеводного обмена при MODY2 диагностировались раньше, чем при MODY3: в 7,8 года (4,0; 10,5) против 11,8 года (9,7; 13,5) (p<0,01). Степень нарушений углеводного обмена была менее выражена при MODY2 у 22,4% пациентов показатели углеводного обмена (уровень HbA1c, гликемия натощак и на 120-й мин перорального глюкозотолерантного теста) были ниже диабетических значений, при MODY3 эти показатели соответствовали диагнозу сахарный диабет в 100% случаев. Пациенты с MODY2 реже получали сахароснижающую терапию. Нарушения углеводного обмена у одного из родителей пробандов были диагностированы раньше при MODY3 — в 24 года (18,5; 35,3) против 32 лет (27; 37) при MODY2 (p<0,05), и родители чаще получали сахароснижающую терапию — в 94,4% против 22,2% соответственно (p<0,01).

Заключение. Данное, наиболее крупное в России, исследование позволяет констатировать более частую встречаемость MODY2 в детском возрасте, более мягкое течение заболевания с менее выраженной дисфункцией β-клеток, чем при MODY3.
